Witness Program

After being assessed as eligible for VAD by two VAD trained doctors, each person must make a written request. The written request must be witnessed by the Coordinating Medical Practitioner and two witnesses. One of the witnesses can be a family member, but the other witness must be independent of the doctor and any institution where the person may be living.

By the time a person becomes eligible for VAD, their illness may have resulted in them becoming isolated from family, friends and the community. They may find it difficult to identify two witnesses for their written request.

In this case, the Coordinating Medical Practitioner can request assistance through the VAD Care Navigators to provide witnesses.

VADSA has a group of caring, compassionate and trained witnesses who are available to witness the written request. We provide people in regional and metropolitan areas.
